Last update### Incident Summary On May 21st, 2024, mobile workers experienced errors on the CommCare mobile application while trying to sync. Web users on CommCare HQ also encountered errors while logging into the web-based applications. ### What Happened The handling of requests from mobile devices and web apps, such as restores, case searches, and form submissions, was blocked at times during the incident. Restore requests, especially large ones, can be resource-intensive, increasing CPU and memory usage. In this particular case, the issue was caused by mobile reports included in some restores. One such restore caused one of our machines to run out of memory, preventing many users on CommCare from syncing to the server and logging into their applications. Fortunately, no data was lost, as the restores completed later the same day after we deployed a fix. ### Looking Forward Our engineering team has conducted a retrospective to understand the root cause of the issue. The team is primarily investigating a long-term fix to improve the restore logic and prevent this incident from reoccurring in the future.

Last update**Incident Summary** On June 8th 2023, CommCare HQ experienced a historically long outage in our form submissions processing service that lasted about 8 hours. Everyone was affected to some degree on [www.commcarehq.org](http://www.commcarehq.org), but those who rely solely on Web Apps or real time form submissions as part of their project workflows were most affected. The issue began around 12:00 PM UTC and lasted until 8:00 PM UTC when we were able to restore services and unlock the ability for our users to submit forms again. For Web Apps users only, during the period of the outage, we regret that the error messages as shown were not clear about whether forms were being accepted or rejected as a result of the problem. Based on this ambiguity, users may have re-entered data in a new form that they previously thought had not been submitted, potentially creating duplicate cases and forms. If you use Web Apps, we recommend reviewing your submission data and related cases for the period of this incident. We would like to stress that there is no concern of data loss or database integrity as a result of this incident. We deeply regret the inconvenience and frustration this has caused for our users. We understand and cherish the trust our users have in our services, and so we appreciate the patience as we worked tirelessly to resolve this issue. **What Happened** Dimagi’s engineering team is currently undergoing a large effort to update the core operating system underlying many of our backend tooling and has been routinely rolling out incremental maintenance over the last few months. In most instances, this is happening smoothly and without issue. We recently updated a service which facilitates the propagation of form and case data to a searchable index. That maintenance resulted in a misconfiguration which was not immediately apparent and which did not disrupt function. Yesterday, when the machine was restarted as part of routine upkeep, the misconfiguration became apparent and immediately hindered its normal functioning. It was no longer able to communicate with other key pieces of our infrastructure. As a result, normal form and case processing was interrupted. Our engineering team responded to the issue right away and began by validating a maintenance procedure that would safely restore functionality with no loss of data. Once a safe path forward was confirmed, our engineering team applied it, immediately resolving the issue. **Looking Forward** We take great care and pride in our maintenance and upgrade procedures, and incidents like this are rare. Our team is undergoing a rigorous retrospective process to understand at a deeper level the underlying technical and process level failures which led to this incident. We will do our due diligence to retrace our steps and have a complete picture of how to prevent this issue from occurring in the future. This includes reviewing all other service configurations for potential issues and updating our maintenance procedures such that an incident with this specific failure mode never happens again.
